A critical problem facing many developing countries is the steady loss of professionals to the developed world, tempted by the higher salaries, better working conditions and professional development opportunities.

This is of particular concern in Africa, where the so-called 'brain drain' more closely resembles a flood. According to the World Health Organization (WHO), 25% of African healthcare professionals work in developed countries. It is said that there are more Ethiopian physicians working in Chicago than in the whole of Africa. The French-speaking part of Africa is equally affected: the IMO (International Organization for Migration) notes that there are more Beninese physicians in France than in Benin.

Since 1990, the continent has been losing 20,000 Africans a year to other destinations. This hemorrhaging of skills means that Africa has to spend about US$ 4 billion every year on employing non-African expatriates instead.

Africa has the lowest ratio of health workers to population in the world (2.17 per 1,000 inhabitants). The ratio is nine times higher in Europe and 19 times higher in the US. Yet Africa also has a severely under-resourced health system and the highest HIV/AIDS burden in the world.
